Severe dysfunction at all levels of management. Arrogance/narcissism of upper management, refusal to clearly communicate company goals and priorities, key personnel lack experience working elsewhere that would provide valuable insight on how a successful company is run. Standard practices and procedures (documentation, procedures, etc.) in place at most other tech firms in Silicon Valley are completely ignored here. Watch out if you're a senior researcher/engineer/tech with experience elsewhere because if you dare suggest approaches that have worked successfully elsewhere, you will be viewed as a threat and singled out harassment by high-profile but hyper-sensitive individuals who have never held a job anywhere else.
