Hierarchical setup makes lots of people middle managers so there are fewer people eligible for unionizing (and more people actively against unionizing). Incompetent management is so far removed from participant-facing staff that many ill-informed decisions are made. Very hard to get hired unless you already know someone there who can vouch for you. COLA is tied to yearly performance review, which is often biased against marginalized staff.
