Minpunten
There are too many tiers of management and issues never get raised to the highest levels. The people in charge don't want to hear about the problems and have been playing a risky game for too long. Some staff in middle management positions are so removed from reality it is scary hence the companies poor performance over the last few years. Still the upper echelons have not learned any lessons.
