Large language models (LLMs) often struggle to identify and appropriately respond to psychiatric emergencies, providing unhelpful or even harmful advice that has resulted in the facilitation of suicide, self-harm, violence, and eating disordered behaviors. Our new study with Harvard Medical School and Beth Israel Deaconess Medical Center introduces the Verily Behavioral Health Safety Filter (VBHSF), a step forward in AI safety for mental health. The VBHSF detects mental health crises in text communications and categorizes them into specific categories of crisis. Our team evaluated the VBHSF on two independent datasets and benchmarked its performance against two state-of-the-art open source general purpose guardrails to test robustness and generalizability. The VBHSF showed strong, well-balanced performance in detecting mental health crises (high sensitivity of 0.990 and specificity of 0.992) and in identifying specific crisis categories (F1-score of 0.939), which largely generalized to an external dataset. It also outperformed open-source content moderation guardrails on sensitivity. The VBHSF demonstrates robust and generalizable performance, prioritizing sensitivity to minimize missed crises – a critical feature for healthcare applications. This research underscores our commitment to developing safe and effective AI for behavioral health.
