Failure of Managers to manage again and again. Relationships and who you know more important than having actual skills......Compensation is on the low side and failure of management to actually communicate leaves a lot to be desired, Same mistakes continue to happen more often than what you would expect for a company of it's size. Where people have taken rolls for the sake of compensation where they neither had the desire or skill to manage.
